SaltW's got a dedicated Minecraft server that's been up and running for a few weeks now. Currently playing is me, Chef and Verne, and we'd like to invite a few more people to play with us. The server only has capacity for 6 people but this should work out pretty well considering timezones. We'll add more slots if we need to at a later point.
The way we've played so far is everyone's created a base for themselves somewhere in the world that represents their playing style, and every once in a while we meet up to exchange resources or invite each other over to show cool stuff or do some dungeon romping. Here are screenshots of everyone's bases so far:

We haven't been playing with rules so far, but it might be good to establish a few now that more people will be joining us:
No griefing/destroying/burning shit. The server takes backup snapshots every 15 minutes, so there's no use doing it.Try to preserve the landscape by not filling it with weird pillars, arrows, blocks, torches, broken structures etc. Make sure your structures have a clear purpose - otherwise the world will eventually end up looking like crud.If you're playing a nomad gameplay style - i.e. you build tiny shelters where you stay until you've depleted nearby resources - destroy your structure when you're ready to move on, and take the building blocks with you. Even if the map is infinite it still has a central starting point where most people will spend their time, and in order for it to last as long as possible we'd like to avoid uninhabited nomad huts littering the landscape.
